People were getting physically ill? I can believe it! FFO used to give me ridiculous amounts of shoulder/neck pain.
When I focus I tend to tense my back muscles hard and lean forward sticking to my uber micro (T_T)
In all seriousness even after switching to a gamepad for FFO *which I played FFO 99% of my total time on a PS2 stick* you have to FOCUS so damn much you rarely have time to relax.
WoW we laugh at moments where "If you don't down Kael'thas to 50% in 1 min he 40k shoots the tank" har har, in FFO the OLD OLD HNMs could drop a meteor that would instantly kill EVERYONE in the area, I mean everyone. And most of their spells are insta-death spells. Not to mention it wasn't an instance and you'd see the other group's NM watchers standing in the back hoping you wipe so they can run in and take the NM. It's a lot of pressure and a shit ton of teeth grinding, NMs would respawns days later some of them even a week.
You didn't tank so much as do a triathalon run with multiple tanks and you pride yourself on kiting the guy for atleast a minute before dying and needing to be reraised. It is that brutal and that different of a game.
That said everything else in FFO operates just like WoW and other MMOs...tank..healer..dps...cc. Until high end raids where your tanks are literally sacrificial lambs and it's not can they tank the mob but how long can they tank to give healers time to raise the tank and throw him back into the tank rotation.
S-E there is a limit to how hardcore one needs to be. A LIMIT! I remember pulling 26hr sessions. Getting a good group might come once a week so when you found one you just stuck it out and pulled pulled pulled. I definitely remember playing with Wade from 7:00am to 9:00am the next day trying to get our last coffer keys.
Brutual MMO from last gen...I'm 100x happier with the casualness of our newer mmos. Although I do sorta miss the hardcore ness of FFO during the end games. Lots of risk but tons of reward as well.

WOAH!!!! Thats nuts. Like Israel said, I miss and love FFO because of its hardcore gamestyle. FFO = MMO bootcamp
